Housing Navigation Assistance
We connect clients, including seniors, individuals with disabilities, the homeless, the chronically ill, and crime victims, to safe, long-term housing and more.
Justice Involved Population
We partner with correctional facilities across California to support justice-involved individuals as they prepare for reentry and reintegrate into their communities.
Alta Regional CFS Services
Our CFS team works directly with members to build daily living skills, strengthen routines, and increase confidence in managing personal care, household tasks, safety, and community connections.
Short-Term Post-Hospital Housing (STPHH)
Short-Term Post-Hospital Housing (STPHH) provides safe, temporary housing along with ongoing support to help individuals recover and stabilize. Services focus on helping members regain daily living skills, access needed medical, mental health, or substance use treatment, and receive case management. Members are also supported in starting the process of finding longer-term housing, including Housing Transition and Navigation Services.
STPHH can also support individuals who have been recently released from jail or prison and need a safe place to recover and reconnect to care.
Eligibility
To be eligible, an individual must:

Have ongoing physical or behavioral health needs, as determined by a qualified health professional, that would otherwise require continued institutional care without STPHH services, or
-
Be recently released from jail or prison and in need of supportive housing and care.
Housing Setting
STPHH may be provided in a private or shared interim housing setting where residents receive the supportive services listed above.
