If you've recently left the hospital, behavioral health facility, substance abuse treatment program or you're getting back on your feet after being released from jail or prison, we're here to help.
Our Short-Term Post-Hospital Housing program gives you a safe place to stay while you recover. During your stay, our team will help you with things like medical care, mental health support and everyday needs. We'll also help you take the next steps toward finding stable, long-term housing.
You don't have to figure everything out on your own, we'll be with you every step of the way.
Who Can Receive This Service?
You may qualify if you:

Have ongoing medical or mental health needs and need extra support after leaving a hospital or other care setting.

Have recently been released from jail or prison and need a safe place to stay while reconnecting with healthcare and other support services.

Depending on availability, you may stay in a private or shared home while you receive support from our team. During your stay, we'll work with you to help you recover, build stability, and prepare for your next chapter.
